The Center for Advanced Rehabilitation at Concord Reserve helps residents achieve their highest possible level of independence. Our therapists develop a tailored treatment plan based on each individual’s needs and capabilities. Services extend to individuals with speech, swallowing, or cognitive disorders, hip or knee replacements, those recovering from surgery or a fracture, as well as individuals managing osteoarthritis and rheumatoid arthritis.
Our rehabilitation professionals work closely with the other members of Lutheran Home’s care giving team in nursing, spiritual life, dining services and social services for an integrated approach to care giving. The individual, their family, and primary care physician are included in the assessment of an individual’s condition and needs. This not only leads to comprehensive, well-managed care, but also provides a reassuring sense of continuity.
Physical Therapists assist our residents in achieving their optimal physical function and mobility. Experienced therapists provide expert treatment for fractures, strokes, balance disorders, arthritic disorders, and orthopedic diagnosis. The Physical Therapy team provides a continuum of personalized care for you or your loved one.
Therapists use state-of-the-art equipment featuring the NeuroCom Balance Master®. This revolutionary computer based module tests the resident’s balance and postural stabilization. Balance can be improved by training for the activities of daily living (such as grocery shopping, stepping up a curb or stairs, and leaning to fold laundry). These activities are simulated in the Balance Master® program. Assessment using the Balance Master®, allows the therapists to enhance therapy treatment by pinpointing any underlying impairments that may hinder the residents function. Because regular physical activity offers substantial improvements in health and well-being, the physical therapy department includes a variety of equipment including a bicycle, a recumbent cross trainers and a treadmill.
Occupational Therapy assists people of all ages to achieve independent and productive lives. Occupational therapists consider “occupation” to be those activities such as taking care of personal needs (self-care), enjoying life (leisure), and contributing to the social and economic fabric of their communities (work/productivity). Occupational therapy works to break down barriers impeding an individual’s ability to function in everyday activities.
Treatments include, but are not limited to, assistance and training for activities of daily living (ADL), including bathing, grooming, dressing, and feeding. ADL training is sometimes supplemented with adaptive equipment to enhance the individual’s independence. Specially designed dining utensils, dressing tools and reaching devices are all examples of tools used to maximize the individual’s ability to function independently. Occupational Therapy also provides treatment for cognition and balance, plus exercises necessary for balancing a checkbook, washing dishes, and folding clothes. Other treatments include specialized static/dynamic splinting, environmental adaptation, and visual perceptual compensation techniques.
| The Occupational Therapy department specializes in assistance with wheelchair mobility and positioning. Therapists work on mobility techniques that improve our resident’s independence while staying at the facility. Wheelchair positioning is also an important aspect in a resident’s care. Proper wheelchair positioning reduces the risk of developing skin breakdown and contractures in older adults. Our licensed speech-language pathologists and treat individuals for communicative, chewing, and swallowing problems. Individualized rehabilitation treatment plans are key to restoring a resident’s highest possible levels of function and ability. Concord Reserve is proud to have been the first nursing home in the Cleveland area to implement both Vital Stim and DPNS, two leading edge advances in therapy for individuals with swallowing problems.

Music Therapy is another part of the comprehensive care that Lutheran Home provides to its patients, residents and their families.
Music therapy is the clinical use of music to address the psychosocial, emotional, physical, cognitive and spiritual needs of those who are well, and those who are suffering from illness. Any family member or interdisciplinary team member can refer a resident or patient to music therapy. Some reasons a patient may be referred to music therapy include:
Music therapy provides an environment where the patient and families can express thoughts and feelings in the comfort of their own space and in a therapeutic setting. The music therapist can meet with the patient on a one to one basis or with family members present. Music therapy can provide extra support and meaning to residents lives and their families.
