Lutheran Home at Concord Reserve is an independent not-for-profit organization that has been dedicated to providing excellence in health care, rehabilitation and senior living services for more than 78 years. Lutheran Home at Concord Reserve is open to individuals of all faiths and is a 192 bed skilled nursing facility that accepts Medicare, Medicaid, Private Pay and several private insurances.

Lutheran Home at Concord Reserve’s campus has developed over the past several years as the needs of our community and residents changed. For example, in January 2008 we opened Main Street , our educational, spiritual and cultural center. Our Health Center was renovated and dramatically transformed the neighborhoods and resident rooms to provide a more comfortable and home-like environment. Nearly all of our rooms are private, feature private bathrooms and beautiful viewsof the landscaped grounds.

In January 2009, Lutheran Home at Concord Reserve opened The Paragon. These comfortable single residences feature private bathrooms with walk-in shower, large screen TV, microwave, refrigerator and organized walk-in closet. The roaring fireplaces and inviting common areas are just the place to read, visit with family and make new friends.

The first floor of the Paragon is our Assisted Living Residence with two different units offering personal services. The Coburg provides a secure atmopsphere offering memory support services and specialized programs that encourage independence. The Augsburg provides services for individuals when they need a hand with their activities of daily living.
