Concord Reserve Assisted Living offers a full range of services for our residents. Apartments are rented month-to-month. Basic services offered to all Concord Reserve residents include:
- All meals and snacks daily
- Weekly housekeeping and laundry
- 24-hour on-site nursing staff
- Emergency response system
- Scheduled transportation
- Life enrichment activity programs
- Flat screen television with over 30 basic cable channels included
- Individual heat and cooling control in suite
- A wide variety of spiritual life programs and services
- Outdoor parking is available to our residents who drive
Personal Assistance
Concord Reserve strives to meet each resident’s needs while maintaining the highest level of choice and dignity. This plan is based on your interests and requirements. It is reviewed periodically and can be changed as needed. Typical care plan services may include:
- Medication management
- Mealtime reminders
- Incontinence care
- Escort assistance to and from meals and activities
- Additional housekeeping services
- Health services: monitoring, supervision or assistance
- Select diabetic care
- Assistance with personal care such as bathing, dressing and grooming
- Periodic nighttime assistance
Additional charges apply based on the personal assistance services provided. Physician services are available through our Wellness Center and rehabilitation services are available through the Center for Advanced Rehabilitation.

Concord Reserve Assisted Living Community is part of the Paragon Building located in the back of our campus. This community’s many features enhance the lives of our residents who enjoy the beautiful great rooms, fireplaces, a courtyard, walking paths, and private rooms with flat-screen televisions.
