Continuity of Care

Our rehabilitation professionals work closely with the other members of Lutheran Home’s caregiving team so that Rehabilitation, Nursing, Spiritual, Dining Services and Social Services are provided as part of an integrated approach to caregiving. The individual, their family, and primary care physician are included to carefully assess the individual’s condition and needs. This not only ensures comprehensive, well-managed care, but provides a reassuring sense of continuity. Adding to our continuity of care, Lutheran Home at Concord Reserve is proud of the fact that all therapists are staff members of the facility, not contracted employees from a therapy company. By being staff members, communication, continuity and coordination with the entire care team is greatly enhanced.
